How to Refer A Patient
Anyone can make a referral, providing the person requiring the care, agrees. Referrals can be done online by clicking the link.
Referrals can be made by the person requiring care, by a relative / carer / loved one, by a GP or another organisation.

Eligibility
Banksia’s services are available to clients of all ages who have a life limiting illness and are living in the Cities of Banyule, Whittlesea and the Shire of Nillumbik.
Care and support is offered to the whole family. Clients can be discharged or readmitted as appropriate.
If you do not live in the catchment area, or would like general palliative care information please call Palliative Care Advice on 1800 360 000.
Cost
The services offered by Banksia are at no cost to clients within our service area.
(There may be associated costs depending on your individual needs, for example Bolton Clarke and some equipment hire costs.)
Banksia can facilitate the supply of equipment for hire, such as toilet seats, recliners, hospital beds and other items.
